| 内容記述 | 分子標的薬による皮疹はQOL を低下させ，重症例では治療中断を余儀なくされるため， 適切なセルフケアが重要である．本研究は，分子標的薬による皮疹を体験した進行がん 患者が受けたセルフケア指導の実態と理解度，および患者のニーズを明らかにすることを 目的とする．症状マネジメントのための統合的アプローチ(The Integrated Approach to Symptom Management：IASM)を研究の枠組みとし，大腸がん，肺がん患者を対象に， 自記式質問紙調査を実施した．質問項目は，基本的知識・基本的技術に関するセルフケア指導を 受けた項目の有無と理解の程度，基本的看護サポートに対するニーズ(自由記述)とした． 質問紙は84 名に配布し51 名から回答を得た(回収率60.7％)．セルフケア指導を受けたと 回答した割合は基本的知識は74.8％と高かったが，基本的技術は63.5％と低かった． セルフケア指導を受けたと回答した割合は，投与経路(点滴＞内服)，治療場所(入院＞外来)に おいて有意差がみられ，理解度は治療場所(入院＞外来)において有意差がみられた． 基本的看護サポートに対するニーズには「看護師に話を聞いてもらい，症状や対処を確認して 安心したい」などがあった．患者が症状をモニタリングするための基本的技術のセルフケア指導が 不足していること，内服治療および外来通院の患者に対するセルフケア指導を強化する必要性が示唆された． |

| 内容記述 | This study conducted a fact-finding survey of the self-care guidance received by cancer patients who experienced skin rash caused by molecular-targeted drugs. Using “The Integrated Approach to Symptom Management: IASM ” as the research framework, we conducted a self-administered questionnaire survey of patients with colorectal cancer and lung cancer. The questionnaire items investigated patientsʼ understanding (4 levels) of selfcare guidance including “essential knowledge,” “essential skills,” and “essential supportive nursing care ” ( free description ). The questionnaire was distributed to 84 people and 51 responses were obtained ( recovery rate 60.7% ). The percentage of those who received guidance on essential knowledge was high, but the percentage of those who received guidance on essential skills was low. There was a significant difference in the rate of receiving guidance on the administration route (intravenous drip＞oral administration) and treatment site ( hospitalization＞outpatient ), and levels of understanding of the treatment site were significantly different (hospitalization＞outpatient). The need for essential supportive nursing care was expressed, such as “I want a nurse to talk to me and check my symptoms and how to deal with them.” There was a lack of self-care guidance on the essential skills required for patients to monitor their symptoms. The study suggested that self-care guidance on oral treatment and outpatient services should be strengthened. |
